Changing Plan Member's Coverage

Adding or Deleting Dependents

  1. Plan Administrator completes Sections 1, 2 and 11 of the Change Form by indicating the Plan Member's ID, name, effective date of change etc. and signs the form
  2. Plan Member completes applicable information in Section 11 and 12 of the Change Form and signs the form
  3. Return a copy to:
