Life Insurance

The Life Insurance plan is available to employees of the member groups of the Ontario Non-Profit Housing Association (ONPHA), their spouses and dependent children.

Optional Life Insurance

Each Plan Member may have different life insurance needs above the amount offered for Basic Life Insurance. Optional Life Insurance allows a Plan Member to purchase additional life insurance through payroll deductions. The cost of this coverage is considerably less compared to the cost of individual insurance outside of a group plan. Medical evidence of insurability is required for this benefit.

Dependent Life Insurance

While Basic and Optional Life Insurance cover Plan Members, the loss of a spouse or child can also result in unexpected expenses or financial burden. Dependent Group Life Insurance is an affordable benefit you can offer your group that can provide the financial assistance Plan Members need to help them through this difficult time.

Insurance Rates

Rates for the Basic Life Insurance and Dependent Life Insurance are based on the specific demographic features of each insured group. The volume purchasing arrangement available to you allows for very competitive rates.

Optional Life Insurance rates are based on the age and smoking status of the Plan Member and/or spouse

Designating a beneficiary

In the event of a Plan Member’s death, benefits will be paid to the beneficiary. A beneficiary may be appointed on the Enrollment Form, or changed through a Change of Beneficiary Form, subject to the law in the province of residence.

What you need to know …

ONPHA groups that offer the Life Insurance need 100% participation from employees in the plan. Plan Members must be under age 70 and residing in Canada.

Medical evidence of good health is required for Optional Life, for late applicants, or when applying for benefits above guaranteed limits. Members and their employees must apply for or have coverage before their spouses can apply.

Accidental Death & Dismemberment

The ONPHA Accidental Death & Dismemberment (AD&D) plan provides a cost effective group benefit plan enhancement that ensures additional financial protection if a Plan Member dies or is seriously injured due to an accident. The coverage provides protection anywhere in the world, 24 hours a day.

An amount equal to 66 2/3% of the amount of insurance for:

loss of speech, or

Optional Accidental Death & Dismemberment (AD&D) Insurance

Similar to Optional Life Insurance, Optional AD&D Insurance can be purchased by a Plan Member through payroll deduction. Optional AD&D provides another means to protect against the financial hardship caused by a severe accident.

A flat, competitive rate for optional AD&D Insurance is available to all member groups.

Designating a beneficiary

In the event of a member’s death, benefits will be paid to the beneficiary. You may appoint your beneficiary on the Application Form, or change your beneficiary at any time by completing a Change of Beneficiary Form, subject to the law in your province of residence. If you do not appoint a beneficiary, or your named beneficiary predeceases you, benefits will be paid to your estate.

STD and LTD by Cooperator's

These programs are designed to protect your employees’ most valuable asset – their ability to earn an income.

Short Term Disability Insurance (STD)

Short Term Disability or Weekly Indemnity insurance, as it is often called, provides Plan Members with protection against short term loss of income due to injury or illness. Your group plan offers Short Term Disability insurance at affordable group rates.

Long Term Disability (LTD)

This plan is available to the ONPHA member groups and their employees. Plan members must be under age 65, working full time (at least 15 hours per week) and residing in Canada. Groups can design their own Long Term Disability plan and determine the level of income replacement offered to Plan Members. Benefits typically commence after 17 weeks of disability, or at the expiration of the Short Term Disability plan, where applicable.

The premium rate for Long Term Disability Insurance considers both the demographic risk of your group and claim experience of the ONPHA LTD pool.

Extended Health Care & Dental by Green Shield Canada

Health and Dental benefits are available to members of ONPHA, their employees, and eligible dependents. Plan Members must be under age 70, reside in Canada, and be covered by their provincial health care plan.

Rates for Health Insurance will vary based on the amount of coverage, and the claim experience of the group. The program allows for a variety of combinations of benefit levels, maximums, dispensing fee or coinsurance levels in order to meet the needs of each group depending on the size of your group.

The flexibility of the program in combination with the volume-purchasing arrangement creates Health Insurance rates that are affordable for any size group.

Survivor Benefit

In the event of your death while insured for these benefits, your spouse and/or dependent children will remain insured for health/dental coverage for up to 2 years from the date of your death.

The volume purchasing arrangement available to you allows for very competitive rates. Premium rates are subject to change upon your renewal date.

Premiums are payable in advance on a monthly basis. You may pay using pre-authorized bank deduction, credit card, or by cheque. Rates shown do not include provincial sales tax, where applicable. Rates are reviewed on a yearly basis. You will be advised in advance of any necessary rate changes.
